Virtual Family Health Team
The Department of Family and Community Medicine in collaboration with the North York Family Health Team and North York Toronto Health Partners OHT have launched a new Virtual Family Health Team. This team will provide virtual primary care assessments for patients who:
1. are experiencing a new medical issue and cannot access their own family physician during the COVID-19 pandemic or do not have a family physician;
2. have been discharged from NYGH for a COVID-related illness.
Patients will also have access to allied health services, including pharmacy, social work/case management, and nutritional support through physician referral. For continuity of care, the team will send documentation from the encounter to the patient's current family physician. For those without a family physician, we will attempt to match them with physicians who are currently accepting patients.
Visits will be conducted through video conference using a secure platform, and will require the use of a patient email to register using the following link:

Providing Virtual Care for your Patients
As many providers are choosing to move towards virtual care options for their patients, OMD has provided guidance on virtual care for clinicians wishing to implement. OTN also has a web page highlighting solutions for primary care providers.
Ontario Health Quality (formerly known as Health Quality Ontario) has also posted guidance on Adopting and Integrating Virtual Visits into Care.
Furthermore, the North York Toronto Health Partners had an online discussion pertaining to implementing virtual care in your practice (March 17, 2020). Click here to view the recording. Dr. David Kaplan,a North York Primary Care Provider, has also developed Virtual Care Hardware Considerations for clinicians.
