Vaccination Program for Homebound Patients
As we continue to vaccinate residents of North York, we are planning a vaccination rollout plan for homebound patients. This ‘Primary-Care Led’ approach -supported by NYTHP, is set up to complement the vaccination plans in place through Home Care and Toronto Paramedics Services. You can find out more information here. Visits for homebound vaccination that have already been submitted to home care or TPS will continue as planned without re-submitting any lists; this is an additional option to leverage the OHT resources.
To participate, we are asking North York physicians to compile lists of their homebound patients (aged 18+) residing in Toronto. The ask is for you to reach out to your eligible patients to identify those wishing to be vaccinated and obtain their consent to share their contact information over password protected email with North York Toronto Health Partners (NYTHP) and partner agencies like Toronto Paramedic Services (TPS) for homebound vaccination scheduling and coordination.
Please collect the information and send it to us over email, using the excel sheet below. Note you will need to password protect the excel sheet before sending it back. Instructions are included below as well.
Please use the following criteria when reviewing your eligible patients:

Patients that have physical, social or mental barriers that inhibit them from visiting a vaccination centre/clinic, pharmacy, hospital or primary care to receive the COVID-19 vaccination. Furthermore, if patients have barriers in transportation and/or strategies to overcome the barriers have been considered but are not available or not appropriate in the person's circumstances (as defined by the TCLHIN)
Please complete and submit the excel spreadsheet with the names and demographic information requested for your interested homebound clients at your earliest convenience to assist in coordinating this process. This will serve as the basis as we start to refine our plan and vaccination strategy. We will ensure we update appropriate partners who may have some of these same patients on their vaccination lists.
