"I wasn’t feeling quite right for a while and recently learned that I have a chronic disease..."

I was always getting sick with bad coughs that never went away and feeling tired all the time. I was told by a doctor that I have chronic conditions which will need to be managed. My partner and I need help to figure out what to do next and how we can get some support.
