Privacy and information management practices at NYTHP
North York Toronto Health Partners (NYTHP) values and respects the privacy rights of the patients and clients it serves. NYTHP ensures all its team members take appropriate steps to protect the privacy, confidentiality and security of patient and client information in accordance with the Personal Health Information Protection Act (PHIPA) and applicable laws.
Information Sharing Agreement
NYTHP has established an Information Sharing Agreement to govern how information is shared among team members, including those who are Health Information Custodians or Agents of Health Information Custodian with equal obligations under PHIPA.
Highlights from the Information Sharing Agreement include:

Each member of NYTHP shall use reasonable physical, technical, and administrative safeguards for the storage and handling of patient and client information that ensures its security and confidentiality are maintained in accordance with PHIPA and consistent with good practice in the health sector.

Patients and clients have the right to withhold or withdraw consent, as appropriate.

No patient or client personal health information will be accessed, used, modified, copied, reproduced, destroyed or disclosed to any third party outside of NYTHP except to the extent necessary to serve the healthcare needs of the patient or client and with the consent of the patient or client or as otherwise permitted or required by law.

All NYTHP team member representatives undergo privacy and security training and are subject to confidentiality agreements.

All NYTHP team members must adhere to the Information Sharing Agreement before any identifiable patient or client information is shared with them.

NYTHP Privacy Governance Structure
NYTHP has a Privacy Working Group that includes privacy leads from the various team members that make up the NYTHP. The Privacy Working Group helps to ensure that NYTHP initiatives comply with PHIPA requirements and the NYTHP information sharing agreement. In addition, the privacy working group supports team members in meeting privacy compliance obligations and implementing the Information Sharing Agreement requirements, where applicable.

Implementing integrated care successfully requires sharing clinical data, collaborating on care plans, and coordinating activities to facilitate better communication with you and your health care team. This leads to better patient experience and outcomes so there may be an impact on how your personal information is collected, used or disclosed.
Information will only be collected on the basis of your consent. You may withdraw consent at any time.
NYTHP commits to collecting and using only that personal information which is necessary to effectively plan for and deliver your health care and health-related services.
