NYTHP OHT shares insights on community initiatives at health care conferences over 2025

North York Toronto Health Partners Ontario Health Team (NYTHP OHT) was proud to present our community-based health initiatives and innovative models in primary care, chronic disease management and preventative care at three recent health care conferences this year.
Each of these presentations reflected our OHT’s dedication to co-designing integrated care solutions with providers and community members, aligning our healthcare strategies with the needs of the neighbourhoods we serve.
Participating partners included physicians, healthcare and community leaders from diverse member organizations including the NYTHP OHT Primary Care Network, North York General Hospital, Flemingdon Health Centre and Working Women Community Centre.
2025 Department of Family and Community Medicine (DFCM) Conference
Edwin Alvarado presented a poster at the DFCM Conference at the University of Toronto, Scarborough and at NYGH’s Education Week in June as part of the Education Scholarship Poster Showcase, which shared first-hand insights on the launch of our innovative digital support service for primary care providers. In his role, our North York Primary Care Digital Support Consultant, Edwin provides members of our Primary Care Network (PCN) with in-person or remote digital support for primary care offices in North York, free of charge.

The poster mapped out how the Digital Support Consultant role was co-designed with primary care partners to address administrative burden in primary care. It also demonstrated how the model was developed based on a needs assessment with physicians and in collaboration with organizations such as the University Health Network (UHN) and OntarioMD to streamline digital tool adoption and increase practice efficiencies. The conference also provided valuable opportunities to engage with peers and gain new insights into streamlining workflows and improving services.

2025 International Conference on Integrated Care (ICIC25)
NYTHP delivered nine presentations at ICIC25 in Lisbon, Portugal, which showcased our ongoing work spanning innovative models in low-barrier health care access, chronic disease management and preventative care.

We were proud to share our emerging model of chronic disease prevention and management and how we have been able to launch this model for those who need it most. Neil Stephens, Director, Systems Transformation, and Teresina Stanichevsky, Lead Community Ambassador, co-facilitated a 60-minute workshop on the hyperlocal impact of our Chronic Disease Management Community Hub, which is being rolled out in high-needs communities across North York.

We also presented our Community Health & Information Fair (CHIF) model with a focus on a low-barrier event where we offered a sexual health clinic for uninsured community members, which included cervical cancer screening, birth control counselling and prescriptions and sexually transmitted infections (STI) counselling, testing and treatment. Teresina Stanichevsky and Adora Chui, Evaluation Scientist, shared insights on overcoming barriers to access through this CHIF. It was also an opportunity to learn from other health care leaders, researchers, clinicians, community representatives and caregivers from around the world who are engaged in the design and delivery of low-barrier integrated care.

In another example of Ontario Health Teams (OHTs) working together, our primary care leads, Dr. Maria Muraca and Dr. Rebecca Stoller, and Sally Moy, OHT Manager, also had the opportunity to co-present sessions with leaders from other OHTs focused on new leadership models in integrated care, improvements to access to preventative care and chronic disease management and supporting initiatives aimed to address Canada’s primary care access crisis.

Canadian Association for Health Services and Policy Research Conference
Sameena Somani, Primary Care Project Assistant, on behalf of the Primary Care-Homecare Initiatives Team (Dr. Adam Dwosh, Colleen Briggs, and Adora Chui) presented a poster at the Canadian Association for Health Services and Policy Research (CAHSPR) Conference in Ottawa, which focused on collaboration across providers as a core pillar of health system transformation.
Our poster showcased findings from a quality improvement project aimed at enhancing the collaboration between primary care providers and home care providers in North York. We were able to conduct and study a test of change to support communication in strengthening our primary care systems, especially when bridging care between primary care and home care providers.

The CAHSPR Conference also reinforced the importance of equity-informed approaches to primary care, which aligns with our OHT’s ongoing work to address the lived experiences of patients, especially those from underserved populations, as well as our commitment to inclusive care.
